I acknowledge and agree that:
• Divine Mercy Home is not responsible or liable for any outcome, injury, loss, or dissatisfaction related to the services of any referred provider.
• I agree to indemnify and hold harmless Divine Mercy Home, its staff, and its volunteers from any legal claims or disputes arising from services delivered by the referred parties.

If I referred to a caregiving company and am dissatisfied with the care received, I understand that Divine Mercy Home cannot be held responsible.

I represent that I am authorized to request this referral on behalf of the care recipient, either because the care recipient has given me permission or because I am legally authorized to act as the care recipient’s representative. I acknowledge this referral disclaimer on my own behalf and, to the extent permitted by my authority, on behalf of the care recipient.
